The purpose of this questionnaire is to determine whether the support staff have observed any changes in cooking skills following the client’s participation in the Healthy Eating Adds Up program. This form contains 15 questions on the observation of participant’s specific skill progress. The observer rate the participant’s different skill with four rating .
